Symposium registration includes: admittance to educational sessions, the exhibit hall, continental breakfast and coffee breaks each day, and a reception on Saturday evening.
Spouse Registration Fee
The Spouse/Guest registration fee includes a continental breakfast, mid-morning refreshments, and Saturday evening’s reception. Spouse/Guest registration does not include admittance to any
scientific presentations or the exhibit hall.
Ways to Register
By Mail: Send completed form (check or credit card payment) to AHS Registration, 19 Mantua Road, Mount Royal, NJ 08061.
By Fax: Send completed form (credit card payment only) to AHS Registration at (856) 423-0082. If you submit your registration by fax, please do not send the original by mail as this may result in duplicate charges to your credit card.
Registration Deadline
Pre-registration will close on Monday, November 15. Please do not mail or fax your registration after that date. If you are unable to register prior to November 15, onsite registration will be available at the conference.
Acceptable Forms of Payment
Checks are acceptable when registration is submitted by mail or completed on-site at the conference. Checks must be payable (in U.S. Dollars) to AHS Scottsdale Headache Symposium® and issued by a U.S. correspondent bank. The registrant is responsible
for any and all bank charges.
Credit Cards may be used for all registrations. AHS accepts Visa, MasterCard, and American Express. AHS reserves the right to charge the correct amount if different from your calculated total. Charges will appear on your statement as “AHS Registration”.
Wire Transfers are accepted provided that the NET AMOUNT of the wire (after bank fees) is sufficient to cover all registration fees in U.S. Dollars. Please contact AHS Registration at (856) 423-0043 for additional information on payments made by wire transfer.
Cash and Travelers Checks (US Dollars only) are accepted for onsite registration at the conference. Please do not send cash or travelers checks through the mail.
Group Discounts
AHS encourages you and your colleagues to take advantage of the discounted fees available when two or more from an organization register for the conference. Registration for group discounts must be submitted together (by mail or fax) with acceptable forms of payment. The discount of $25.00 will be applied after the first registration is charged the full registration amount. The group discount is not available through online registration.
Not an AHS Member?
Discounted registration fees are one of the many benefits of becoming a member of the American Headache Society®. If you are not an AHS member, and you submit a completed membership application along with your registration form (on or before October 23), you will receive the AHS Member registration rate. A membership application is available on the website, or by calling (856) 423-0043 and requesting one be sent to you.
Confirmations
Registrations will be acknowledged with a written confirmation within two (2) weeks of receipt. Please be sure to provide a valid email address so that we may send your confirmation electronically.
Cancellation Policy
Requests must be submitted in writing to AHS Registration, 19 Mantua Road, Mount Royal, NJ 08061 and postmarked no later than Monday, October 18. All fees will be refunded minus a $50.00 processing fee. THERE WILL BE NO REFUNDS AFTER THE OCTOBER 18 DEADLINE.

In accordance with the guidelines of the Ethics Committee for Continuing Education of the American Headache Society®, presenters for this program have been asked to identify any personal financial relationships which, in the context of the subject of their invited presentation, could be perceived as real or apparent conflict of interest. Current guidelines state participants in CME activities should be made aware of any affiliation or financial interest that may affect the speaker’s presentation(s). Each speaker has been requested to complete a conflict of interest statement. The names of faculty members declaring a potential conflict of interest and those who declined to sign a statement will be indicated in the conference syllabus. Additionally, financial disclosure information for all speakers is provided in slide format and projected immediately prior to each speaker’s presentation.
